Unicorn blood for sure, but it is possible that the other ingredients are not included in the recipe (...if that's the proper thing to call the ingredient list for a potion). I don't think it ever gave the full ingredient list in the book.Unicorn blood!

EDIT: In the first book, the Sorcerer's Stone, Snape tells Harry on page 138 that "asphodel and wormwood make a sleeping potion so powerful it is known as the Draught of the Living Dead".
